Credentialing and Additional Services
Spring Forward OT is covered under your Medicare Part B or your Outpatient Therapy Insurance Coverage. We are currently credentialed with the following insurance providers:
Medicare (NE and IA)
BCBS of Nebraska
Aetna

Spring Forward OT is currently seeking credentialing with several other commercial and Managed Medicare providers.

If your insurance provider is not listed above:
I will try my best to work with any plan that is not currently contracted as in-network. Clients with out-of-network benefits may be eligible for reimbursement. Additionally, private pay options are available. Please reach out to discuss your options further. For your convenience, I accept cash, checks, and all major credit cards for copayments and private fees.​
Additional Services
In addition to therapy services that are covered by your insurance company, Spring Forward OT provides other therapist-led services. A few examples include home modification consultations, one on one prehabilitation consultations, group prehabilitation meetings, and general wellness education. Associated costs vary based on service.
Intake Paperwork
Once you have set up an appointment for an initial evaluation, you'll receive an email with an invitation to a secure Client Portal. Once you're signed up, you can fill out intake paperwork through this portal. Should you not have easy access to a computer or the internet, alternate intake paperwork will be arranged for in order to most conveniently meet your needs.
