Living In Place
When where you dreamed about living is where you end up...

At any stage of life...

Regardless of circumstance and life events...

To allow you to safely do what you want, where you want.

That is the point of Living in Place
It's more than Aging In Place
Living in Place is more than just an idea. It is a framework.
It is about living the life you want, where you want, and through all life changes.

It is a way of looking at where you live and carry out activities.
It is acknowledging that the environment plays a crucial role in how we live.
It is a way of framing therapy and healthcare in such a way so as to enable you to live life safely and happily in the environment that you prefer.
It is for all ages.
Many people know of this concept as Aging in Place; however, I believe that the benefits expand far beyond aging. For one, more people are living longer and through more extreme circumstances. Thus, individuals of any age can be facing a mismatch between their environment and their day to day function. Secondly, we all have aspects of our home that don't quite meet our needs. For these reasons, I feel that the term Living in Place tends to best explain these concepts.

Now more than ever, after over a year of a pandemic, more people are choosing either to stay at home longer or keep their loved ones at home, rather than send them to live in an assisted or skilled nursing facility. Some individuals may also choose to move in with other family members. In all of these cases, an occupational therapist can work with individuals and their loved ones to support everyone's needs in relation to their unique environment and situation.

For some, staying at home is no longer an option or not something that they desire. That is valid and there is often not a right or wrong answer. Healthcare professionals knowledgeable in the Living in Place model can help you to navigate those questions for yourself or your family.

To truly focus on Living in Place, you need a team of people.
Spring Forward OT, as an occupational therapy provider, is one piece of the puzzle.

Ok, so what does a Living in Place model for therapy really look like?? It is:

-
Traditional outpatient occupational therapy to help you physically and cognitively as needed, but done at home
-
Practice and training actually doing the activities that you are most concerned about, including the incorporation of safety, prevention, and wellness
-
Training family in strategies (transfers, ways to physically or verbally assist, etc)
-
Education on medical equipment, home modifications (as needed), or just DIY modifications
-
Training on alternative daily activity management strategies
-
A consultation (once or ongoing) to look at your environment with you and your family
-
Support - Having a therapist to advocate for your needs and answer some of your questions

Living In Place
Living life where you imagined, with safety and happiness.
Peace of mind for you and your family .
